Pomerene and Holmes Health Department to conduct community health assessment

Pomerene Hospital and the Holmes County Health Department will be conducting a community health assessment this February. Over the next two months nearly 1,200 adults living in Holmes County will be receiving a health survey by mail. Any adult in Holmes County may receive this survey. The surveys will arrive in a large white envelope containing a survey booklet, a preaddressed and prestamped envelope to return the completed survey, and a $2 bill for participation.

The anonymous survey asks participants to answer questions about general health, risk and protective health factors, and access to health care. For those that receive and complete the survey, all answers will be anonymous and confidential. Only group results will be analyzed. The results of these surveys are very important in helping local agencies identify key health problems and needs of Holmes County residents.

The release of the final health assessment report will be in the fall of 2017.

Pomerene Hospital and the Holmes County Health Department encourage those who receive a survey to participate. This assessment will help local health organizations plan health programs based on the needs of the community. For those receiving and completing a survey, mail responses shortly after receiving the survey in the mail so data can be compiled in a timely fashion.

For questions about the survey call Jennifer McCoy at the Holmes County Health Department at 330-674-5035 or email her at jmccoy@holmeshealth.org or call Tara Martin at Pomerene Hospital at 330-473-3781 or email her at tmartin@pomerenehospital.org.
